The purpose of the referral process is to provide effective and timely support to initiate an appropriate plan of action for students at their individual level of need.

How do I refer a child to visit with the counselor?

A student's physical, emotional, and social well being are incredibly important and a priority for me as the school counselor. If you have a concern for a student, please fill out the correct referral form below and we can start the referral process.

My number one priority is to help students when they have a need and this is where referrals come in. Click this link to learn about the referral process.

Parent Referral

If you are a parent and you feel your child needs to visit with the school counselor, please fill out the form in this link. If this is an URGENT need, please contact my office immediately at 417-466-7512 and I will get in touch with you promptly. If you feel a child is in danger, could potentially harm themself or harm someone else, please hotline immediately at 1-800-392-3738. You may continue with the referral process after the child is out of any immediate danger.

Faculty Referral

If you are a teacher, staff member, or administrator and you feel a student needs to visit with the school counselor, please fill out the form in this link. If this is an URGENT need, please contact my office immediately at 417-466-7512. If you feel a child is in danger of hurting themself or someone else, please hotline immediately. You may contact me after a hotline call has been made to continue with a referral and document the hotline call.
